NHM Dhamtari Invites Application for 38 Staff Nurse and Various Posts

Candidates must apply in person on the designated walk-in date. Applications sent by post/courier will not be accepted.

1. Download Application Form: Download the prescribed form from the Dhamtari district website (www.dhamtari.gov.in) or collect it from the CMHO office notice board.
2. Fill the Form: Fill the form carefully and affix a recent passport-size photograph.
3. Attach Documents: Arrange and staple the following in the specified order:
   • Application Form
   • Demand Draft (Application Fee)
   • Educational Qualification Certificates & Mark Sheets (8th onwards)
   • Technical Qualification Certificates
   • Professional Council Registration Certificate
   • Domicile Certificate
   • Caste Certificate (if applicable)
   • Disability Certificate (if applicable)
   • Experience Certificates (if claiming marks)
   • Employment Registration Certificate
   • Photo ID Proof
4. Report on Time: Report to the Office of the CMHO, Dhamtari, on the designated registration date between 9:00 AM and 11:00 AM.
5. Registration & Objection: Complete registration. Check the eligible/ineligible list displayed by 5:00 PM. Raise any objections between 5:00 PM and 5:30 PM.
6. Attend Interview/Test: Report the next day at 10:00 AM for the final eligible list, followed by the selection process from 11:00 AM.

Important: Separate applications and Demand Drafts are required for each post if applying for more than one. No TA/DA will be provided.

• Walk-in Only: Candidates must appear in person on the designated date for their post. No online or postal applications are accepted.
• Eligibility Check: Ensure you meet the educational qualification, age, and professional registration requirements for the specific post.
• Application Fee: Pay the correct fee via Demand Draft/Banker's Cheque. The application is incomplete without it.
• Documents: Bring all original certificates and self-attested photocopies for verification. Applications without the required documents will be rejected.
• Experience Marks: Claim experience marks only with relevant government/semi-government experience certificates. Experience will be calculated at 2-4 marks per year, up to a maximum of 5 years.
• No TA/DA: No travelling allowance will be paid for attending any part of the recruitment process.
• Contractual Basis: The engagement is purely contractual up to 31 March 2027 and is extendable based on performance.
