District Health Society Giridih Invites Application for 108 Staff Nurse, Pharmacist and Various Posts

Candidates must apply offline by following the steps below. Applications must be sent by Registered/Speed Post only.

1. Download Application Form: Obtain the prescribed application format (Annexure) from the official notification.
2. Fill the Form: Complete all details correctly, including the post code on the application and the envelope.
3. Attach Documents: Attach self-attested photocopies of all required documents:
   • Educational qualification certificates and mark sheets (including CGPA conversion chart if applicable).
   • Caste certificate (if claiming reservation).
   • Date of birth/Matriculation certificate.
   • Local residence certificate.
   • Experience certificates (if applicable, volunteer experience is not valid).
   • Valid driving licence (if required).
   • Two passport-size colour photographs (one affixed to the application).
   • A self-addressed envelope of size 24x10 cm.
   • Demand Draft for the application fee.

• Contractual Engagement: This is a purely contractual appointment under NHM, valid for one year, extendable based on performance. It does not amount to regular government employment.
