• Blank application forms will be available from 21 January 2026 To 29 January 2026 (Except Saturday & Sunday) between 10:30 AM to 1:30 PM and 2:00 PM to 5:00 PM.
• Location: Office of the Executive Health Officer, Public Health Department, 3rd Floor, F/South Ward Building, Dr. Babasaheb Ambedkar Road, Parel, Mumbai-400012.
• Applications must be submitted at Room No. 50, same address.
• Applications sent by post will not be considered.
• Backward Class candidates must submit attested copies of Caste Certificate, Caste Validity, and Non-Creamy Layer certificate.
